Description

American Traveler is seeking a Home Health Physical Therapist with 2+ years of home health experience and HCHB/Oasis charting proficiency.

Details

Home health setting covering geriatric patients with cases including start of care and follow-up visits
Caseload of 6-7 patients per day, potentially 3 visits worth of points
Minimum productivity requirement of 30 points per week
Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
No weekend requirement unless needed to meet weekly productivity targets
No on-call requirements
Territory spans White, Cumberland, and Van Buren Counties with a 50-75 mile coverage radius
Mileage reimbursed at IRS standard rate
Uses HCHB and Oasis for charting

Requirements

Minimum 2 years of home health experience required
Active state PT license required (license in hand, no pending licenses accepted)
License required if applicable
Current BLS certification required
HCHB and Oasis charting experience required

Additional Information

Dress code is scrubs or business casual; PTs wear a polo with scrub pants
First-time travelers will not be considered for this position
Local candidates are accepted
