Description
American Traveler is seeking a Physical Therapist for Home Health with HCHB and OASIS experience to serve a geriatric patient population.
Details
- Home health setting covering the Mossyrock & Morton Area
- Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
- Caseload of approximately 5-7 patients per day
- Patient population is geriatric
- On-call required every 6th weekend
- Uses HCHB (Homecare Homebase) EMR system
- Traveler expected to be flexible with territory assignments
Requirements
- BLS certification required
- Minimum 1 year of recent relevant experience required
- HCHB (Homecare Homebase) proficiency required
- OASIS experience required for completing Start of Care assessments
- Active driver's license and personal transportation required
- License required if applicable
- Timely documentation and communication required
Additional Information
- Responsibilities include home exercise programs and patient/family education
- Treats conditions including orthopedic, neurologic, and post-surgical cases
- Monday orientation required, including completion of billable modules
- Hospice/Home Health checklist completion is mandatory
- Scrubs required in any color, no jeans permitted
- iPad provided; cell phone not provided
- Mileage reimbursed at the IRS standard rate
- First-time travelers will not be considered for this role
- Local candidates will not be considered
- Travel pairs accepted, but each traveler must have their own territory and transportation
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
- Requested time off must be submitted at the time of application
