Description
American Traveler is hiring an RN for a PACE Home Care position working with the geriatric population in Battle Creek, MI, requiring an active MI RN license.
Details
- PACE (Program of All-Inclusive Care for the Elderly) home care setting serving older adults as an alternative to nursing home care
- Part of an interdisciplinary care team including physicians, nurse practitioners, social workers, therapists, and aides
- Geriatric patient population with acute and chronic medical conditions
- Monday through Friday schedule, 8:00 am to 4:30 pm
- Includes regular home visits for care and treatment as ordered by the PCP
- On-call responsibilities as needed, including weekends and holidays
Requirements
- Active MI RN license required
- Associate degree required; BSN preferred
- Minimum of one year of experience working with frail, elderly, or long-term care populations
- Home care and wound care experience preferred
- Current CPR certification by AHA required, or willingness to obtain
- Proficiency with Microsoft Word and Outlook required
- Current valid Michigan driver's license and vehicle insurance required
- Reliable transportation required for travel between centers
- Current immunizations required (Hepatitis B, TB, MMR, Varicella, Tdap, Flu, COVID-19) or medical waiver documentation
Additional Information
- Provide direct nursing care including wound care, medication administration, immunizations, lab draws, and assessments
- Triage emergent situations and manage complex clinical situations independently
- Collaborate with licensed and non-licensed staff, participants, and families using a participant-centered care model
- Document in participant medical records per policy
- May be exposed to infectious waste, diseases, and hazardous materials as part of job duties
- Subject to callback during emergency conditions such as severe weather or disasters
- Must be able to push, pull, move, sit, or stand for long periods and lift up to twenty pounds
- Must be 18 years of age or older
