Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent home health PT experience and a valid CA PT license.

Details

Home health setting with outpatient clinic exposure
Day shift, 5x8-hour schedule from 8:00 AM to 5:00 PM
Open to 10-hour shifts or 4-day work weeks with flexible scheduling
Caseload of approximately 4-5 patients per day (5 weighted visits)
Patient population includes geriatrics, orthopedics, and neurology cases

Requirements

CA PT license required if applicable
Minimum 1 year of experience as a Home Health Physical Therapist required
Valid driver's license and auto insurance required
BLS certification preferred
Experience with home exercise programs, patient and family education, and safety/injury prevention
Familiarity with OASIS documentation and home health charting preferred
Skilled in assessment, gait training, wound/skin integrity checks, and use of adaptive and assistive devices
Experience treating orthopedic, neurologic, and cardiopulmonary conditions preferred

Additional Information

Responsibilities include patient evaluation, treatment planning, home exercise program instruction, and documentation of care
Treats a variety of conditions including post-op orthopedic cases, stroke recovery, and chronic pain management
Works independently in patients' homes while coordinating care with the broader care team
Accepts local candidates at the local rate
Maximum of 7 days RTO per contract, with only 1 holiday approved for time off per contract
