Description

American Traveler is seeking a Physical Therapist for a home health position requiring BLS certification and at least 1 year of experience.

Details

Home health care setting covering patients within the designated service area
Common diagnoses include CHF, orthopedic conditions, and pneumonia
Responsibilities include start of care, resumption of care, recertifications, evaluations, and daily visits
Expected to complete approximately 6.5 points per day/week
Uses Homecare Homebase (HCHB) for charting
Schedule is Monday-Friday, 8a-5p with rotating weekends
Weekend coverage follows a rotating schedule, 8a-5p
Participation in holiday rotation required
40 hours per week

Requirements

Current BLS certification required
Minimum 1 year of experience preferred; new graduates not preferred
Active state or compact (NLC) license required for the state of practice
License required if applicable
Copy of driver's license required for consideration
Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and rehire eligibility

Additional Information

Clinical manager or educator available 24/7 for emergencies, and during business hours for general support
Orientation includes two weeks with a preceptor and a few visits with a work coach
Scrubs or business casual attire accepted
Phone and tablet provided for the role
Open to first-time travelers, though prior home health experience is preferred
