Description

American Traveler is hiring a Home Health RN with a BSN and at least one year of acute care experience to provide care coordination and nursing assessments for adults with disabilities and older adults in home and community-based settings.

• Care provided in home and community-based settings
• Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
• Flexibility of hours required
• EPIC charting system experience required

Requirements

• Active, unrestricted CA RN license issued by the California BRN with current NURSYS in hand at time of consideration
• Current BLS certification issued by AHA
• BSN required
• Minimum one year of clinical nursing experience required, preferably with geriatric populations
• At least 1,000 hours of acute care hospital experience required
• Minimum one year of travel nursing experience required
• Recent EPIC experience required
• Valid driver's license with proof of current, permanent address required
• Two professional references required at supervisor level or above (no charge nurses accepted)
• Proof of vaccinations required for consideration

Additional Information

• Provides case finding, consultation, health-focused assessments, and care coordination services for clients
• Develops and certifies care plans in accordance with program mandates and clinical practices
• Provides counseling and supportive interventions within scope of practice
• Coaches and educates unlicensed caregivers at licensed Assisted Living Waiver facilities to ensure quality care
• Manages caseload productivity and resource allocation in accordance with program spending guidelines
• Participates in at least one departmental committee and quality assurance activities
• Represents the department in the community through outreach, referrals, and partnership events
• Local candidates accepted within a 75-mile radius of the facility
• Maximum RTO of 7 days required at submission; no RTO accepted afterward
