Description
American Traveler is hiring an experienced RN Case Manager for an acute care hospital setting requiring 2 years of direct case management experience and an active CA RN license.
Details
- Acute hospital Case Management department
- Day shift, 5x8 hours, 8:30 AM to 5:00 PM
- 13-week assignment
- Patient ratio of 1:20
- Complex patient population with chronic or complicated medical conditions
Requirements
- Active CA RN license required at time of submission
- BLS certification required
- ACM and Interqual certifications preferred
- Minimum 2 years of direct acute care Case Management experience required
- Any charting system experience accepted
- Community hospital experience preferred
- 2 supervisor references preferred
Additional Information
- Conduct initial patient assessments and ongoing stay reviews
- Collaborate with interdisciplinary team to evaluate patient needs and create comprehensive discharge plans
- Coordinate discharge to appropriate level of care for complex patients
- Must reside within 75 miles of the facility
- RTO requests always require facility approval and must be submitted upfront
- First-time travelers not considered for this role
