Description
American Traveler is hiring an RN for a Home Health position requiring a UT or compact RN license and at least one year of home health, hospice, or acute care experience.
Details
- Home Health setting serving adult and pediatric patients in their home environment
- Four 10-hour day shifts per week, Monday, Wednesday, Friday, with every other Saturday and Sunday
- Requires ability to commute to the office twice daily to electronically transfer patient files
- Uses Epic for charting
Requirements
- Active UT or compact RN license required
- Current BLS certification (AHA) that does not expire before start date
- ASN or Nursing diploma required
- Minimum 1 year of experience in home health, hospice, or acute care setting preferred
- Valid driver's license with reliable transportation and acceptable driving record
- Ability to transfer and assist patients independently, including lifting, bending, and navigating stairs or uneven surfaces
- Basic computer skills and strong verbal and written communication skills
- DOB and last 4 of SSN required for consideration
Additional Information
- Assess, plan, implement, and evaluate individualized care plans for home health and hospice patients
- Coordinate and collaborate with interdisciplinary care teams, including physicians and support staff
- Deliver care in compliance with hospice/home health policies, legal, and regulatory standards
- Promote patient and family education and prepare patients for transitions in care
- RTO must be submitted with candidate profile
- Returning Intermountain employees must be separated from the facility for at least one year before being eligible for a travel assignment
