Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring at least 1 year of skilled home health experience and OASIS/HCHB proficiency.

Details

Skilled Home Health setting with 1:1 patient ratio
Day shift schedule, 8-hour shifts (8:00 AM - 4:30 PM)
Approximately 30 points per week productivity, up to 6 patients per day
Weekend support may be required
No on-call requirement
Uses Home Care Home Base (HCHB) for documentation, Remote Client for Home Health on laptop, Rover phone application, and OASIS documentation

Requirements

Active Oregon license required before start date if applicable; temporary licenses not accepted
Current BLS certification required
Minimum 1 year of skilled Home Health experience required
OASIS experience required
Home Care Home Base (HCHB) proficiency required
Must have reliable personal transportation and valid driver's license

Additional Information

Main responsibilities include conducting skilled home health visits and completing OASIS documentation as needed
Facility groups patient visits to minimize driving, though service area may extend up to 60 miles or 60-90 minutes of driving per day
Mileage reimbursed at current IRS standard rate
Business casual attire required; no jeans; scrubs permitted but not required with no specific color
Free parking available
Up to 3 days of RTO may be automatically approved, with blackout periods during Thanksgiving, Christmas, and New Year's weeks
Open to first-time travelers
Accepts local candidates within a 50-75 mile radius at the same rate
