Description

American Traveler is hiring an experienced RN Case Manager for a 13-week acute hospital assignment requiring a CA RN license and at least 2 years of case management experience.

Details

• Acute hospital setting within the Case Management department
• Supports MS, SDU/PCU/IMC/Obs, and Surgical Services patient populations
• Day shift schedule, Monday through Friday (8:30 AM - 5:00 PM)
• Every other weekend required, with every weekend preferred
• Holiday coverage required
• Allscripts charting system experience required

Requirements

• Active California RN license required
• Minimum 2 years of case management experience required
• Experience in care coordination, discharge planning, and disease management
• Experience with needs assessment and DME ordering
• Knowledge of CMS, Department of Health, HIPAA, OSHA, and Joint Commission/Core Measures/National Safety Goals regulations
• References required for consideration
• Copies of required certifications must be included for consideration
• Must pass an advanced EKG exam onsite (20 questions, 45-minute time limit, 80% minimum passing score, one attempt only)

• Responsible for care coordination, discharge planning, and disease management for assigned patients
• Business casual attire required, scrubs not necessary
• RTO must be submitted with initial application and cannot be added once submitted
• No RTO permitted during the first two weeks of the assignment
