Description
American Traveler is hiring an experienced RN Case Manager for an acute care hospital role requiring at least 12 months of acute care case management experience and EPIC proficiency.
Details
- Care Coordinator role within an acute care hospital setting
- Caseload of 16-25 patients per day per care coordinator
- Day shift schedule, Monday through Friday, 8:00 AM to 4:30 PM
- No weekend rotation required
- 13-week contract assignment
- Uses EPIC EMR system
Requirements
- Active and clear NY RN license required
- BLS certification required
- PRI and Screen certification preferred
- Minimum 12 months of acute care nursing experience required
- Minimum 12 months of acute care case management experience in a hospital setting required
- EPIC experience required
Additional Information
- Responsibilities include acute care case management, discharge planning, and utilization management
- This role operates differently than other case management positions, making relevant acute care CM experience essential to success
- Travelers receive 3 days of training and are expected to function at full capacity thereafter
- Business casual attire or scrubs permitted
- Local candidates residing within a 50 mile radius of the facility are eligible for a local rate
