Job Title: Physical Therapist (PT) - Home Health |Winter Haven, FL (Local or Travel)

Location: Myrtle Beach, SC
Employment Type: Contract (13 Weeks)
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (40 hours/week)
Guaranteed Hours: 0, full time available

M-F, 8am - 5pm, 40 hours per week

13 weeks

On-call info: On-call required if requested- on-call needs are subject to change based on staff and branch needs
Guaranteed Hours: 0, full time available
Productivity Units: 30 units
Qualifications: HCHB, Oasis, and HH experience required

Position Overview

We are seeking an experienced Physical Therapist (PT) for a Home Health assignment based in Winter Haven, FL . This position involves travel across multiple counties and offers consistent weekday hours.

Qualifications

• Experience:

  • 1-2 years as a Physical Therapist in an acute care or rehabilitation setting - Required

  • Previous Home Health experience - Preferred

  • HCHB (Homecare Homebase) EMR experience - Preferred

• Licenses & Certifications:

  • Current State License as a Physical Therapist - Required

  • Current CPR certification - Required

  • Valid driver's license with reliable transportation and insurance - Required

• Education:

  • Bachelor of Science in Physical Therapy from an APTA-approved program - Required
