Description

If you want to make a significant impact as a Registered Nurse - Case Manager in Waukesha, WI this position might be what you are looking for!

Responsibilities:

Coordinate and oversee patient care plans to ensure optimal health outcomes and efficient use of healthcare resources.
Assess patients’ medical conditions, social determinants of health, and care needs to develop individualized care plans.
Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care.
Coordinate insurance authorizations, appeals, and approvals for necessary medical services and treatments.

Qualifications:

WI Registered Nurse license
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Basic Life Support (BLS) certification
Case Management Certification (CCM) or Accredited Case Manager (ACM) certification preferred
