Opportunity:
Occupational Therapist - Home Health
Job Details:
• New graduates are welcome to apply
• Home health experience preferred but not required
• Assigned territory covers Area #1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City) and either Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont) or Area #3 (San Ramon, Dublin, Pleasanton, Sunol, Livermore)
• Two scheduling options available: Guaranteed Hours or Non-Guaranteed Hours
• Guaranteed Hours option requires a minimum productivity of 27 points per week with a 36-hour guarantee and 30 points completed per week
• Guaranteed Hours option requires willingness to travel outside the assigned area or work weekends to maintain weekly visit expectations
• Guarantee does not apply during the first and last week of assignment, weeks with holidays, or weeks with approved time off
• Non-Guaranteed Hours option may require weekend coverage
• Non-Guaranteed Hours option may occasionally require travel outside the service area to maintain a full caseload
• Payment is made on a per-visit basis; visit types convert to points and hours for daily hour approval
• Visit types include OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/Up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/Up (1 point = 1.33 hours)
• Rate is all-inclusive and covers drive/travel time, visit time, documentation time, and time spent on phone calls with patients and doctors
• Required credentials: Current CA OT License, BLS, Driver's License, Auto Insurance, Auto Registration, TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual), Drug Screen, and Physical
