Opportunity:

Physical Therapist (PT)

Job Details:

• Coordinate physical therapy services and provide direct therapeutic care to assigned clients of all ages
• Develop and revise individualized plans of care based on initial assessments, client history, diagnosis, physician orders, and expected outcomes
• Provide preventive and rehabilitative physical therapy procedures tailored to age-specific needs
• Perform and monitor treatments utilizing physical agents such as heat, water, and electricity using equipment such as whirlpool baths, electrical muscle stimulation, and ultrasound
• Teach rehabilitation skills to clients and/or caregivers to promote maximum independence and safety
• Formulate and implement appropriate teaching plans and communicate significant clinical findings to physicians and involved personnel
• Document all skilled and medically necessary care thoroughly, accurately, and in a timely manner in compliance with Medicare, State, Federal, JCAHO, insurance, and payer requirements
• Conduct PTA and Home Health Aide supervisory visits and prepare written instructions as applicable
• Supervise Physical Therapy Assistants and Home Health Aides in accordance with Federal and State laws, rules, regulations, and agency policy
• Participate in staff meetings, case conferences, business development meetings, and customer service programs
• Participate in Quality Management programs and complete all required in-service programs to ensure standards are met and skills remain current
• Maintain client and organizational confidentiality at all times
• Demonstrate leadership competencies through communication, relationship building, and teamwork
