Opportunity:

RN - Case Management

Job Details:

• Work 8-hour shifts Monday through Friday from 8:00 AM to 5:00 PM
• Perform case management and coordinate discharge planning for age-appropriate in-patients with chronic diseases and special healthcare needs
• Maintain tracking system for referred patients for admission with other healthcare facilities
• Conduct chart reviews for requested services for return to facility or discharge home
• Work across multiple units including Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit
• Attendance at staff meetings required when scheduled
• Minimum 2 years of experience in discharge planning and Med-Surg required
• RN experience in a clinical, medical/surgical, acute, and/or critical care setting required
• Valid and unrestricted Registered Nurse license in any U.S. State or Territory required
• Valid American Heart Association (AHA) Basic Life Support (BLS) certification required and must be maintained
