We are seeking Registered Nurses interested in transitioning from inpatient care to home-based clinical practice. Whether your background is in critical care, telemetry, stepdown, ER, or med-surg, this role allows you to work independently while still being fully supported by a collaborative, interdisciplinary team.

What You’ll Do

• Deliver skilled, patient-centered nursing care in the home environment

• Conduct assessments, develop individualized care plans, and evaluate outcomes

• Collaborate with physicians, social workers, and therapists to coordinate care

• Educate patients and families to promote comfort, safety, and independence

• Utilize evidence-based practice and technology to support clinical decision-making

Qualifications

• Active RN license in Pennsylvania

• Minimum 15 months of professional nursing experience (acute care strongly preferred)

• BLS/CPR certification (AHA)

• Reliable transportation, valid driver’s license, and auto insurance

• BSN or ASN from an accredited nursing program
