RN Case Management Homecare Job Details

• Location: Yazoo City, MS (serving Yazoo & Holmes County)
• Position: Registered Nurse – Case Management – Homecare (Travel Contract)
• Employment Type: 13-week travel nursing contract
• Start Date: November 17, 2025
• Estimated Weekly Pay: $1,884 - $2,094
• Hours per Week: 40
• Shifts per Week: 5 (8 Hour Days: 8:00 AM – 4:30 PM)
• On Call: Required at least every 3 weeks
• Setting: Home Health, Case Management

RN Case Manager Qualifications

• Active, unrestricted Mississippi RN license or compact state nursing license
• Current CPR certification (required)
• Minimum 1 year of recent RN homecare or home health experience
• Experience with HCHB (Homecare Homebase) preferred
• Proficient in OASIS assessments, wound care, foley catheter care, IV therapy, patient teaching, and comprehensive nursing assessments
• Associate's or Bachelor's Degree in Nursing (ADN or BSN)
• Case Management Certification (CCM or ACM) preferred
• Strong clinical, analytical, and decision-making skills

RN Case Management Homecare Responsibilities

• Conduct comprehensive assessments of high-risk patients in the home health setting
• Develop, implement, and monitor individualized care plans in collaboration with primary care providers
• Coordinate cost-effective, appropriate levels of care and facilitate care transitions
• Arrange non-medical support services (housing, transportation) to support patient compliance
• Work closely with physicians, social workers, discharge planners, and claims professionals
• Engage specialty healthcare resources as needed for optimal patient outcomes
• Maintain accurate records of clinical, functional, and fiscal outcomes
• Promote health education, illness prevention, and patient advocacy in the homecare setting
• Ensure patient privacy and confidentiality at all times
• Identify, prevent, and promptly intervene in adverse patient events
