Case Management RN Job Details

• Location: Tuba City, AZ 86045 (Arizona healthcare jobs)
• Assignment Type: Travel Registered Nurse
• Healthcare Setting: Emergency Department (ED) / Hospital
• Start Date: December 15, 2025
• End Date: March 16, 2026
• Contract Duration: 13 weeks
• Shift: Variable 10-hour shifts (7:00am – 5:30pm), Sunday through Saturday, with weekend rotation and call
• Hours per Week: 40
• Estimated Weekly Salary: $2620 - $2690

Registered Nurse Qualifications

• Associate's degree in Nursing (minimum); BSN preferred
• Active, unrestricted Registered Nurse (RN) license in any U.S. state, territory, or Puerto Rico
• Current Basic Life Support (BLS) certification by the American Heart Association
• Minimum 2 years of recent case management or care management experience in an Emergency Department
• At least 2 years of direct patient care experience (hospital, home health, or skilled nursing facility)
• At least 1 year of travel nursing experience preferred
• Proficiency with Electronic Medical Records (EMR), Microsoft Outlook, Word, and Excel
• Case Manager Certification (CCM or ACM) required within 2 years if not already certified
• Fingerprint clearance required prior to start date

Case Management Nurse Responsibilities

• Coordinate and facilitate interdisciplinary care for Emergency Department patients, ensuring appropriate level of care and efficient patient throughput
• Collaborate with patients, families, physicians, and community resources to optimize care plans and discharge planning
• Assess patients for admission, observation, or outpatient status using established criteria (e.g., InterQual)
• Develop and implement safe discharge plans, including referrals to appropriate levels of care
• Work closely with other case managers, social workers, and clinical staff to support quality care and cost-effective outcomes
• Support patients with mental health needs, chronic pain, and frequent utilization issues by connecting them to healthcare resources
• Participate in the development of clinical practice guidelines and order sets to improve care quality
• Provide care management for patients of all ages, from newborn to geriatric
