Registered Nurse Job Details

Registered Nurse Qualifications

• Active Registered Nurse (RN) license in Pennsylvania
• Minimum 1 year of clinical nursing experience (homecare or home health experience preferred)
• Strong clinical assessment, patient care, and communication skills
• Ability to work independently and manage time effectively in a homecare environment
• Reliable transportation for home health visits within assigned Philadelphia zip codes

Homecare RN Responsibilities

• Deliver skilled nursing care to patients in their homes throughout Bala Cynwyd and Philadelphia (zip codes 19104 & 19139)
• Conduct patient assessments, develop individualized care plans, and administer treatments as prescribed
• Educate patients and families on health management, disease prevention, and medication administration
• Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
• Accurately document all patient care, assessments, and communications in a timely manner
• Participate in scheduled weekend rotation (1 out of every 4 weekends)
