• Contract Duration: 13 weeks
• Hours per Week: 40 (Guaranteed)
• Shift: 8-hour days (8:00am – 4:30pm, flexible start/end times)
• Shifts per Week: 5
• Weekends: Every other weekend
• Holidays: Rotating schedule
• On-Call: 1-2 times/week and every other weekend (triage calls)
• EMR: Epic
• Mileage Reimbursement: $0.70 per mile
• Estimated Weekly Salary: $2100-$2260

RN Qualifications

• Active Registered Nurse (RN) license in Wisconsin
• BLS certification (American Heart Association)
• Minimum 1 year of nursing experience; homecare or case management experience preferred
• Strong clinical assessment and critical thinking skills
• Valid driver's license and reliable transportation
• Ability to work independently and within an interdisciplinary healthcare team
• Excellent communication and organizational skills

RN Homecare Responsibilities

• Coordinate and manage patient care in a homecare setting, ensuring high-quality, patient-centered outcomes
• Conduct comprehensive nursing assessments and develop individualized care plans
• Educate patients and families on disease management, medications, and home safety
• Collaborate with physicians, therapists, and other healthcare professionals for continuity of care
• Document patient interactions and care activities accurately in Epic EMR
• Participate in on-call rotation, triaging patient calls and determining appropriate interventions
• Adhere to all regulatory and compliance standards for Wisconsin nursing jobs
