RN Case Management Job Details
- Location: Summit, New Jersey (07901-3533)
- Employment Type: Travel Contract
- Healthcare Setting: Inpatient Case Management
- Shifts per Week: 5
- Shift Duration: 8-hour days (8:00am – 4:00pm)
- Schedule: Monday – Friday; no weekends, no on-call
- Hours per Week: 40

Registered Nurse Qualifications
- Active RN license in New Jersey
- Minimum 1 year of clinical experience in hospital patient care or case management
- Strong communication, negotiation, and conflict resolution skills
- Proficient in data analysis and computer systems
- Knowledge of state and federal healthcare regulatory standards for case management
- Experience with clinical performance improvement or rapid cycle change
- References and skills checklist required
- Preferred: Acute care case management experience
- Preferred: Case management training from a professional healthcare organization
RN Case Management Responsibilities
- Coordinate patient care and services to ensure safe, timely transitions to appropriate levels of care
- Assess, plan, implement, monitor, and evaluate care options for individual health needs
- Collaborate with healthcare teams to address clinical, psychosocial, and financial patient needs
- Develop and document comprehensive case management and transition plans
- Identify high-risk patients and refer to social services as needed
- Implement strategies to prevent readmissions and analyze causes of readmission
- Monitor patient length of stay and participate in care rounds to meet clinical targets
- Communicate with patients, families, and care teams to facilitate seamless care transitions
- Maintain current knowledge of healthcare regulatory agency rules and regulations
- Educate patients, families, and healthcare staff on care progression and safe transitions
- Identify and address risk management and quality improvement issues
