RN Case Management Job Details

• Location: Summit, New Jersey (07901-3533)
• Employment Type: Travel Contract

• Schedule: Monday – Friday; no weekends, no on-call

Registered Nurse Qualifications

• Active RN license in New Jersey
• Minimum 1 year of clinical experience in hospital patient care or case management
• Strong communication, negotiation, and conflict resolution skills
• Proficient in data analysis and computer systems
• Knowledge of state and federal healthcare regulatory standards for case management
• Experience with clinical performance improvement or rapid cycle change
• References and skills checklist required
• Preferred: Acute care case management experience
• Preferred: Case management training from a professional healthcare organization

RN Case Management Responsibilities

• Coordinate patient care and services to ensure safe, timely transitions to appropriate levels of care
• Assess, plan, implement, monitor, and evaluate care options for individual health needs
• Collaborate with healthcare teams to address clinical, psychosocial, and financial patient needs
• Develop and document comprehensive case management and transition plans
• Identify high-risk patients and refer to social services as needed
• Implement strategies to prevent readmissions and analyze causes of readmission
• Monitor patient length of stay and participate in care rounds to meet clinical targets
• Communicate with patients, families, and care teams to facilitate seamless care transitions
• Maintain current knowledge of healthcare regulatory agency rules and regulations
• Educate patients, families, and healthcare staff on care progression and safe transitions
• Identify and address risk management and quality improvement issues
