Job Opportunity: RN Cath Lab (Travel)

Location: Elyria, Ohio
Facility: Elyria Medical Center
Hospital System: University Hospitals
Contract Length: 13 weeks


Key Requirements and Credentials

Certifications (Required)

  • BLS: American Heart Association only. Required for clinical positions.
  • ACLS: American Heart Association only.
  • License/Certificate Verification: Valid permanent RN license required. Temporary licenses NOT accepted. Must be verified within 30 days of start and updated every 2 years with RN license renewal.

Personal Health History (Required)

  • MMR: Positive titers within last 15 years. Booster protocol applies if low titer.
  • Fit Test: Completed within 1 year of start, updated annually. Specific approved mask types required.
  • Hepatitis B: Proof of full series, partial series with declination, or positive titers within 15 years. No childhood vaccines accepted.
  • Varicella: Positive titers within last 15 years. Booster protocol applies if low titer.
  • Tdap: Proof of vaccination within past 10 years, no declinations accepted.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Negative test within last 6 months. Annual requirement. If positive, clear chest x-ray and current TB questionnaire required.
  • Physical/Health Statement: Pre-employment physical exam within one year of start stating ability to work without restrictions.
  • Drug Screen: 10-panel drug screen within 60 days of start. Instant drug screens not allowed.

Personal Health History (Optional/Preferred)

  • COVID-19 Vaccine: Proof or approved exemption preferred but not required.
  • Influenza Vaccine: Required Oct-March or approved medical/religious exemption. No declinations allowed.
  • Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: If applicable.
  • Annual Negative PPD: If 2-step PPD not done within past year.

Professional Requirements

  • UHS I-9 Attestation: Proof of eligibility to work in the U.S.
  • UHS Ohio Fingerprints: Ohio BCI&I check and FCRA-compliant national criminal background check required. Attestation allowed if fingerprints pending.
  • UHS Education Verification: Highest level of education verified. International education requires equivalency documentation.
  • UHS Employment Verification: Verification of employment within last 5 years through third-party.
  • UHS Criminal Background Check Results: Completed within 6 months of start, including SSN trace and national criminal sweep.
  • UHS OIG Search: Completed within 6 months prior to start.
  • UHS SAM/EPLS/GSA/OFAC: Completed within 6 months prior to start.
  • UHS Module Completion Certificate: Completion of safety and compliance training modules prior to clearance.
  • UHS Competency Exam: Must score at least 80%, completed within 1 year prior to start.
  • UHS Core Competencies: 80% passing score required, completed within 1 year prior to start (includes HIPAA, Joint Commission, OSHA topics).
  • UHS Badge Photo: Professional headshot required.
  • Driver's License/State ID: Non-expired photo ID required.

Additional Requirements

  • UHS Skills Checklist: Clinical skill-specific checklist completed within 1 year prior to start (initial hire only).
  • Submission Packet: Resume including education and work history, 2 professional references within last 2 years, and completed job application required.
