Job Requirements
Experience
- Minimum 24 months of current Level I Trauma ICU experience (required)
- Travel experience preferred
Certifications
- Must have active American Heart Association (AHA) or American Red Cross certifications at time of submission, including:
- ACLS (AHA)
- BLS (AHA)
Licensing
- Active Arizona RN license required at time of submission
- License verification must be less than 30 days old
- No license hits allowed
- Compact license allowed but must be active
Other Requirements
- Candidates with an Arizona home address on their driver’s license will not be considered
- Flu vaccination required; declinations can be applied for (vaccination status must be noted in submission)
- Banner has specific background and compliance requirements that must be met and attested to immediately
- Return to Office (RTO) requests:
- Not approved within the first week or on holidays
- Cannot exceed 7 days
- All learning modules must be completed prior to assignment start
- Non-billable hours adjusted to 16 hours per assignment
- Assignment may be canceled with 7 days written notice without penalty
Submission Requirements
- Fully completed Banner Cover Page including:
- All facilities and departments candidate is willing to work in
- All shifts candidate is open to working
- Resume detailing all facilities, facility size, bed count, trauma level, and units worked
- Skills checklist specific to the discipline (must be less than 1 year old)
- Active license verification (less than 30 days old)
- Current certifications included in submission
- Copy of driver’s license (must not show AZ home address)
- Flu vaccination status included in submission notes
Important Notes
- Start date should be on or very soon after the order start date
- Candidates must be pre-closed and ready to accept the assignment
- Notify promptly if candidate withdraws or becomes unavailable
