Job Summary

The Case Manager I supports physicians and interdisciplinary teams to facilitate patient care, aiming to improve clinical outcomes and patient satisfaction while managing care costs. Responsibilities include utilization management, patient education, care facilitation, and discharge planning. The role involves managing a designated patient caseload, planning to meet patient needs, managing length of stay, and promoting efficient resource use.

Key Responsibilities

• Facilitate precertification and payor authorization processes
• Collaborate in managing patient care across the continuum, removing barriers to timely care and reimbursement
• Apply process improvement methodologies to evaluate care outcomes
• Support and coach clinical documentation efforts; serve as a clinical resource for coders to ensure accurate documentation of illness severity and service intensity
• Coordinate communication with physicians
• Demonstrate knowledge and skills to provide appropriate patient care
• Understand principles of growth and development related to different life cycles

Required Skills & Experience

• Strong interpersonal communication and negotiation skills
• Analytical, data management, and computer skills
• Organizational and time management skills with the ability to prioritize multiple tasks independently
• Ability to exercise sound judgment in interactions with physicians, payors, patients, and families

Preferred Skills & Experience

• Minimum of three years RN clinical experience preferred
• Knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement preferred

Education & Licensure

• Required: Bachelor of Science in Nursing (BSN)
• Required: Current and valid Registered Nurse license in the State of Michigan

Contract Length

• 13 weeks
