Description

Job Opportunity: RN Case Manager

Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Schedule

  • Monday – Friday, 8:00 AM – 4:30 PM
  • Weekend rotation: 1 out of every 4 weekends

Job Description

  • Provide skilled care and case management to patients and families under the direction of the Clinical Team Manager.
  • Work within established organizational and departmental policies and procedures.
  • Practice in accordance with the State Nurse Practice Act and accepted professional standards.
  • Utilize nursing theory and processes to deliver care.
  • Communicate and document observations and assessments.
  • Promote and maintain health through direct care, teaching, counseling, and rehabilitative measures.

Required Credentials & Compliance

Licenses & Certifications

  • RN License: Primary source verification required within 30 days of start; MA Conditional Licenses not accepted.
  • BLS Certification: Required before onboarding (AHA preferred; Red Cross accepted).
  • Driver’s License/State ID: Required before onboarding.
  • Resume: Must include past 7 years of work experience with city and state; required for submission.
  • References: Two recent references required, at least one supervisor or above in the skillset.
  • Skills Checklist: Required for submission.

Health & Immunizations

  • Fit Test (N95): Required within 365 days of start and updated annually; no religious exemptions.
  • MMR: Documentation of vaccination or immunity required before onboarding.
  • Influenza Vaccine: Required during flu season (Sept 15 – April 30); no exemptions.
  • TB Test: Documentation of TB skin test or blood test within 3 months of start; updated if >6 month break.
  • Health Statement: Physician statement within 365 days of start; updated if >6 month break.
  • Respirator Medical Clearance Attestation: Required if fit test is required.
  • Hepatitis B: Strongly preferred but optional.
  • Tdap: Preferred but optional.
  • Positive PPD - Negative Chest X-Ray: Required if TB test positive (optional).

Background & Compliance Checks

  • Criminal Background Check: Within 45 days prior to start; updated if >6 month break.
  • Massachusetts CORI Check: Within 30 days prior to start; updated if >6 month break.
  • MVR Check: Required for all home care positions; within 45 days of start.
  • National Sex Offender Check: Within 30 days of start; updated annually.
  • Exclusion Checks: Within 30 days of start and annually; includes OIG, GSA, MassHealth & NH Medicaid lists.
  • Education Verification: Verified through background check company before onboarding.
  • SSN Trace: Must be run with criminal check covering last 7 years.

Additional Requirements

  • MGB Badge Photo: Color photo taken within past 6 months; no selfies or filters; plain background.
  • Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
  • Candidate Submission Coversheet: Required for submission.

This position requires adherence to strict credentialing, health, and compliance standards prior to and during employment.
