- Positive PPD: Requires clear chest x-ray and annual TB questionnaire; chest x-rays valid for 5 years
- Fit Test: Completed within 1 year of start, cannot be done onsite
Background and Employment Verification
- Criminal Background Check: Must include Social Security Trace, County searches for last 7 years, National Criminal Sweep including Sex Offender registry; valid within 6 months
- Ohio Fingerprints: Required for Ohio residents (5+ years or less than 5 years with FBI check); attestation allowed for pending results
- Employment Verification: Verification of employment within last 5 years through third-party; attestations not allowed
- Education Verification: Highest level of education verified through third-party; international equivalency required if applicable
- Social Security Trace: To be completed within 1 year if not included in background check
- National Database Criminal Background Check: Required for Ohio residents to meet fingerprint requirements
Additional Compliance and Testing
- Drug Screen: 10-panel drug screen within 60 days of start; instant drug screens not accepted
- UHS Competency Exam: Must score at least 80%, completed within 1 year prior to start
- UHS Core Competencies: Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required, completed within 1 year prior to start
- Physical/Health Statement: Pre-employment physical exam within 1 year stating ability to work without restrictions

- UHS Module Completion Certificate: Completion of required safety and compliance training modules prior to clearance; renewal annually
