Job Description

Experience Requirements

• Specialty: Radiology / Cardiology

• Negative TB test or chest X-ray within last 12 months required; if positive, clearance letter required
• Immunizations required:

  • Rubeola (Measles): 2 vaccines or positive titer or approved exemption
  • Mumps: 2 vaccines or positive titer or approved exemption
  • Rubella: 2 vaccines or positive titer or approved exemption
  • Hepatitis B: Series, titer, or declination
  • Varicella: 2 vaccines, titer, history of chickenpox, or approved exemption
• Seasonal flu vaccination required during flu season (10/1-3/31) with hard deadline for documentation
• COVID vaccination preferred (initial series)

Facility Specific Requirements

• Photo for badge: Recent, professional headshot meeting specific guidelines
• Confidentiality agreement, scrub colors, safety brochure, dress code policy, drug-free policy, and hand washing hygiene policy must be reviewed and signed prior to start
• TB questionnaire required annually
• References: 2 professional references required

Orientation & Competency

• Competency exams (medication/pharmacology and others) required as applicable, must be completed within one year of start date
