Job Opportunity: Respiratory Therapist

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• Hours: 06:30 AM to 07:00 PM
• 36 hours per week
• Every other weekend required

Required Certifications & Licenses

• BLS (American Heart Association)
• ACLS (American Heart Association)
• Current Respiratory Therapist license with verification dated within 30 days of start
• Fit Test
• Specialty-specific competency testing (must be completed within the last year)

Required Health & Immunizations

• COVID-19 vaccine with booster by September 2025, or approved declination/exemption (declination not accepted in CA or IL)
• Influenza vaccine or approved declination/exemption (declination not accepted in CA or IL) for current flu season
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start (titers not accepted)
• Varicella: Proof of vaccination or IgG titer lab report
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Tdap vaccine (declinations accepted if no pediatric interaction)
• Physical/Health Statement confirming fitness to work (exam within last year)
• Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)

Professional & Compliance Requirements

• Signed Trinity Code of Conduct Form
• Trinity I9 form completed by agency
• Confidentiality and System Security Agreement signed
• Integrity and Audit Services Form signed shortly before start
• Non-Employee Upload Form completed (specific to Iowa)
• Conflict of Interest Disclosure Form signed
• Criminal Background Check (Federal, Nationwide, County for last 7 years) dated within 30 days of start
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) report dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• Joint Commission CORE Exam within the last year
• NSO (National Sex Offender) check dated within 30 days (Iowa-specific registry checks required)
• System Access Form completed and uploaded one week before start
• Skills Checklist provided by agency for clinical contractors
• Child and Adult Dependent Abuse Training (Iowa only)
• OFAC (Office of Foreign Assets Control) sanctions check completed within 30 days
