Job Opportunity: Registered Nurse - Home Health (Travel/Contract)

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the home clinic; typical weekly travel is 100-150 miles.
• Charting and documentation proficiency required in HomeCare HomeBase (HCHB) and Oasis.
• Schedule: Monday to Friday, 8 AM to 5 PM, with some weekend rotation and on-call requirements (varies by location).

Requirements

Experience

• Minimum 1 full year of recent specialty experience in Adult Home Health or Hospice.
• First-time travelers not accepted; must have recent specialty experience.
• Prior travel experience not explicitly required.

Certifications

• Current CPR and BLS certification required.

Licensure

• Must have an active nursing license in hand (no pending licenses accepted).

Skills

• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months.
• RNs must be proficient in Oasis documentation.

Work Environment

• Dress code: Scrubs.
• Patient visits primarily in home health settings.
• Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 shifts Monday-Friday.

Compliance and Documentation Requirements

• Valid Driver’s License/State ID.
• Background checks including state-specific and federal screenings.
• Education verification.
• Drug screening (10-panel).
• Immunizations and health documents including:

  • Physical exam within 12 months.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination.
  • COVID-19 vaccination or declination.
• Various state-specific background and fingerprinting requirements depending on assignment location.
• Completion of compliance training, confidentiality agreements, and policy acknowledgments prior to start.
• References: Two professional references required.
• Work history verification for the last 7 years or at least two most recent employers.

Additional Notes

• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
• Locals accepted at the same rate.
• Weekend and on-call requirements vary by location and will be discussed during the interview.
• Contractor may be floated to nearby clinics if patient census is low to maintain hours.

This position is ideal for experienced Registered Nurses with a background in home health or hospice care who are comfortable with travel within a defined territory and have strong documentation skills.
