Job Requirements

  • NIH Stroke Certification (valid for 1 year)
  • Dysrhythmia/EKG Competency Exam (Relias or Prophecy Dysrhythmia/EKG Exams only, must be renewed annually)
  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Pass/Fail Testing (EKG/Medication) acknowledgment required for RNs & LPNs

Compliance & Background Checks

  • Government-issued photo ID required (no paper copies except temporary receipt)
  • I9 and E-Verify documentation required
  • Background check within 6 months covering all locations lived/worked in past 7 years, updated annually
  • EPLS/GSA/SAM and HHS/OIG checks required
  • Education verification and accreditation confirmation required
  • NURSYS verification for RNs
  • Primary source verification of state license
  • References: 2 professional references within 2 years of start required for travel positions
  • Employment application and work history verification (2 workplaces within last 7 years) required

Health & Safety Requirements

  • Physical exam within 1 year confirming ability to meet essential job functions
  • Negative TB test (PPD, GOLD, or TSPOT) within 1 year; pending results allowed prior to start
  • TB Questionnaire updated annually
  • Immunizations:
    • 2 MMR vaccines or positive titer (boosters if negative)
    • Hepatitis B titer, series, or signed declination
    • 2 Varicella vaccines or positive titer (boosters if negative)
    • Seasonal flu vaccination or approved medical/religious exemption during flu season (Oct 1 - Mar 31)
  • Negative 9-panel drug screen within 30 days of start (instant screen allowed as placeholder)
  • Respirator fit test to be completed onsite (not required for non-clinical roles)

Facility-Specific Requirements

  • Stroke CEUs: Must be stroke-related, updated annually, required prior to or within one week of start
  • Trauma CEUs: 16-hour initial trauma course (TNCC accepted), renewals annually after 27th week of work
  • Unit-specific skills checklist required
  • Radiation badge required for Radiology Technologists, Interventional Radiology RNs, and Cath Lab RNs (provided by facility)
  • Floating policy and time & attendance policy for contracted staff must be completed and signed

Additional Notes

  • Candidate must complete and sign consent to release information
  • Orientation safety manual acknowledgment and HIPAA compliance required
  • EKG exam is time-sensitive; candidates must pass within 3 attempts (2 attempts for some units) or risk cancellation
  • Education verification and transcript/diploma upload required, especially for out-of-state or international education
  • Some certifications and exams must be renewed annually and verified through specific vendors or platforms
