Job Details

• Years of Experience Required: Minimum 1 year recent experience in Hospice OT
• Prior Travel Experience Required: No (First timers not accepted)
• Average Patients per Day: 5-7 (to be confirmed during interview)
• Guaranteed Hours: 40 hours per week
• Types of Cases: Geriatric patients
• On-call Requirement: Yes, every 6th weekend
• Territory: Up to a 60-mile radius from the hub/office
• Locals Accepted: No
• License: Must have license in hand
• Certifications Required: BLS (Basic Life Support)
• Dress Code: Scrubs (any color), no jeans
• Technology Provided: iPad provided, cell phone not allowed
• Mileage Reimbursement: Yes, at IRS standard rate
• Special Requirements:

  • Flexibility with territory expected
  • Timely documentation and communication required

Compliance & Background Requirements

• Background checks required within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1M
  • Nationwide Sex Offender
  • Motor Vehicle Records (MVR)
  • Education Verification
  • Work History Verification
  • State-specific searches as applicable (e.g., Kentucky, Virginia, Indiana)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting required (Ohio BCI and FBI prints within 12 months of start, updated annually)
• Education verification of highest degree related to profession
• State license primary source verification within 90 days of start and upon expiration
• NURSYS verification for applicable professions (RN, LPN/LVN, AP)
• Competency exams as applicable (Medication Test/Pharmacology, Specialty Competency)
• Agency background attestation required prior to start

Health & Safety Requirements

• Physical exam within last 12 months (not required annually)
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire required
• Immunizations/vaccinations required or documentation of declination:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9 Panel Drug Screen within last 12 months
• Flu seasonal vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Documentation & History

• 2 professional references required (travel, allied, per diem acute care) within 2 years of start
• Full 7 years of work history documented with no gaps, including city and state of employment
• Work history verification within 90 days of start (3 attempts required if unverifiable)
• Annual skills checklist required

Additional Facility-Specific Requirements

• Proof of current auto insurance required for candidates with driving responsibilities (all home health positions)
• Completion of Acceptable Use Policy prior to start
• Facility-specific flu and COVID declination forms if applicable
