Location: Milwaukie, Oregon

  • ACLS (Advanced Cardiac Life Support)
  • BLS (Basic Life Support)
  • PALS (Pediatric Advanced Life Support)
  • NRP (Neonatal Resuscitation Procedure)
  • Chemotherapy certification (if applicable)
  • Electronic Fetal Monitoring (EFM) certification (if applicable)
  • State professional license (copy required)
  • IV Certification (if applicable)
  • Management of Assaultive Behavior (MAB) (if applicable)
  • NIH Stroke Certification (renew every 2 years)
  • CPI (Crisis Prevention Institute) (if applicable)
  • National Tech Certification verification (as applicable)
  • NURSYS verification for RNs/LPNs/APs

Background and Compliance Requirements

  • Background check within 45 days of start, covering all locations lived/worked in past 7 years
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
  • Education verification required for all candidates
  • State-specific searches (Oregon and Washington) as applicable
  • Drug testing: 10-panel drug screen within 90 days of start (no instant tests)
  • References: Minimum 2 supervisor references covering 1 year of employment within last 3 years (6 months for CNAs)
  • Employment application covering at least last 5 years (7 years for APN/Subcontractors)
  • Skills checklists as applicable, updated annually

Health and Safety Requirements

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit mask testing (annual, travel only)
  • Immunizations and titers:
    • TDAP (within 10 years or declination accepted)
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer or declination
    • Hepatitis B vaccination or signed declination
    • Varicella - 2 vaccines or positive titer or declination
    • Flu vaccine during flu season or declination
    • COVID vaccine or declination
  • Respiratory safety clearance (annual for travel)

Competency and Training Requirements

  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Color Blind Testing (if required by unit)
  • Competency exams as required by specialty
  • Specialty-specific competency exams (e.g., Medication Aide exam for CNAs in LTC Oregon facilities)
  • Providence WorkRamp modules:
    • Core regulatory modules annually
    • Specialty-specific modules for ER, L&D, leadership roles, hospice/home health/assisted living
    • Regional and facility-specific modules as applicable
  • Epic training modules required for charting system proficiency

Additional Notes

  • Float policy acknowledgment required (may be required to float to other Providence facilities nearby)
  • Badge photo required (passport style with neutral background)
  • Some certifications and requirements may be deferred or adjusted based on clinical track or facility policies
  • Non-clinical or remote workers may have reduced requirements for some certifications and health documents

This position is ideal for CT Technologists with at least 1 year of experience in Radiology/Cardiology, looking for a travel contract in a short-term acute care setting with no on-call or weekend requirements. The role requires compliance with Providence Health and Services' comprehensive credentialing, health, and training standards.
