Required Qualifications
- Specialty: Radiology / Cardiology (CT Tech)

Compliance & Documentation Requirements
- Background Check: Must be completed within 45 days of start, covering all locations lived/worked in past 7 years, updated annually
- Drug Testing: 10-panel drug screen within 90 days of start (no instant tests), negative results required
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations & Health Documents:
- TDAP (Tetanus/Diphtheria/Pertussis) within 10 years or declination
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Flu vaccine during flu season or declination
- COVID vaccine or declination
- Fit Mask Test: Required for travel candidates, updated annually
- Respiratory Safety Clearance: Required annually for travel candidates
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment Application: Must include at least last 5 years of employment with month/year format, gaps explained
- Skills Checklist: Required for applicable units, updated annually
Facility & System Specific Requirements
- Must complete Providence Health and Services required WorkRamp training modules prior to start, including:
- Annual Regulatory Requirements
- Specialty Specific Modules (if applicable)
- Regional Core and Specialty Modules for Oregon
- Epic training modules required for charting system access
- Photo for badge: Passport style with neutral background required
- Float Policy Acknowledgement: Required (may be required to float to other Providence facilities within reasonable distance)
