Job Opportunity: CT Technologist (Travel)

Location: Maywood, Illinois
Employment Type: Travel
Shift: 12-hour shifts, 8:00 AM – 8:30 PM, including every third weekend
Contract Length: 12 weeks

Required Qualifications

• License: ARRT-CT certification required

• Certifications:

  • BLS (American Heart Association certification)
  • Fit Test
  • Trinity 10 Panel Drug Screen (specific drug panel required)
  • COVID-19 Vaccine (proof of booster, declination or exemption; declination not accepted in IL)
  • MMR vaccination or immunity proof
  • Hepatitis B vaccination series (titers not accepted)
  • Influenza Vaccine (proof or declination; declination not accepted in IL)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) for TB screening
  • Varicella vaccination or immunity proof
  • Physical/Health Statement (fit to work exam within last year)

• Professional Requirements:

  • License/Certificate Verification (must be dated within 30 days of start)
  • Trinity Code of Conduct Form
  • Trinity I9 form
  • Trinity Confidentiality of Patient & Proprietary Information & System Security Agreement
  • Trinity Integrity and Audit Services Form
  • Trinity Motor Vehicle Report (MVR) showing valid license
  • Trinity Conflict of Interest Disclosure Form
  • Criminal Background Check (Federal, Nationwide, County from last 7 years, SSN verification)
  • Education Verification (highest academic degree or diploma/GED)
  • EPLS (Excluded Parties List System) check
  • GSA/OIG Exclusion check
  • Trinity Joint Commission Testing (CORE Exam within a year)
  • NSO (National Sex Offender) check
  • Trinity System Access Form (must be completed and uploaded one week before start)
  • Trinity Skills Checklist (provided by agency for clinical contractors)
