Job Opportunity: Physical Therapist (Outpatient) - Travel/Contract
Location: Ganado, Arizona

Job Description
- Start ASAP
- Schedule:
- 1 week inpatient AM, outpatient PM
- Remaining 3 weeks outpatient only, 8 AM - 5 PM, Monday to Friday
- Schedule details can be discussed during the interview
- Requires experience in both outpatient and inpatient settings
Required Qualifications
- License: Valid Physical Therapist license for Arizona
- Certifications:
- ACLS (Advanced Cardiac Life Support) - American Heart Association or American Red Cross
- BLS (Basic Life Support) - preferably AHA or American Red Cross
- PALS (Pediatric Advanced Life Support) - preferred
- NRP (Neonatal Resuscitation Program) - required for ER (if applicable)
- Background Checks:
- Standard 7-year background check including SS trace, county searches, nationwide, and sex offender registry
- Annual EPLS/GSA/SAM and HHS/OIG checks
- Education verification (copy of highest degree required)
- Fingerprinting required if working with vulnerable populations (confirm with manager)
- Health Requirements:
- Physical exam within 1 year of start
- Immunizations and titers as applicable:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB test or chest X-ray within 1 year
- Measles, Mumps, Rubella (MMR) proof or titer
- Hepatitis B titer or vaccine series (declination not accepted)
- Varicella immunity or vaccination
- Seasonal flu vaccination or facility-specific declination
- COVID-19 vaccination or approved exemption with facility forms
- 10-panel drug screen within 1 year of start and onsite drug test during orientation
- Competency Exams: Medication test/pharmacology competency as applicable
Experience Requirements
