Job Description

• Start ASAP
• Schedule:

  • 1 week inpatient AM, outpatient PM
  • Remaining 3 weeks outpatient only, 8 AM - 5 PM, Monday to Friday
  • Schedule details can be discussed during the interview
• Requires experience in both outpatient and inpatient settings

Required Qualifications

• License: Valid Physical Therapist license for Arizona
• Certifications:

  • ACLS (Advanced Cardiac Life Support) - American Heart Association or American Red Cross
  • BLS (Basic Life Support) - preferably AHA or American Red Cross
  • PALS (Pediatric Advanced Life Support) - preferred
  • NRP (Neonatal Resuscitation Program) - required for ER (if applicable)
• Background Checks:

  • Standard 7-year background check including SS trace, county searches, nationwide, and sex offender registry
  • Annual EPLS/GSA/SAM and HHS/OIG checks
  • Education verification (copy of highest degree required)
  • Fingerprinting required if working with vulnerable populations (confirm with manager)
• Health Requirements:

  • Physical exam within 1 year of start
  • Immunizations and titers as applicable:

    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Negative TB test or chest X-ray within 1 year
    • Measles, Mumps, Rubella (MMR) proof or titer
    • Hepatitis B titer or vaccine series (declination not accepted)
    • Varicella immunity or vaccination
    • Seasonal flu vaccination or facility-specific declination
    • COVID-19 vaccination or approved exemption with facility forms
  • 10-panel drug screen within 1 year of start and onsite drug test during orientation
• Competency Exams: Medication test/pharmacology competency as applicable
