Description
POSITION: Social Worker – Case Management (Acute Rehab)
LOCATION: Washington, DC (Must live 50 miles from the facility)
Shift & Schedule

Shift Time: 8:30 AM – 5:00 PM

Days: Monday – Friday
Required Experience
-
Minimum 1 year of hospital-based case management experience
-
Strong experience with discharge planning
-
Cerner EMR knowledge required
Licensure & Certification (Required)

LICSW

Active DC Social Worker License
Job Responsibilities
-
Provide comprehensive Social Work services in accordance with licensure, certification, training, and regulatory requirements

Coordinate and facilitate services for patients during and after admission to the acute rehabilitation unit

Perform discharge planning, care coordination, counseling, and resource management to ensure safe and effective transitions across the continuum of care

Collaborate with interdisciplinary teams to deliver cost-effective and resource-efficient care

Respond effectively to new and changing work assignments while maintaining high standards of patient advocacy and compliance
