Description

POSITION: Social Worker – Case Management (Acute Rehab)
LOCATION: Washington, DC (Must live 50 miles from the facility)

Shift & Schedule

  • Shift Time: 8:30 AM – 5:00 PM

  • Days: Monday – Friday

Required Experience

  • Minimum 1 year of hospital-based case management experience

  • Strong experience with discharge planning

  • Cerner EMR knowledge required

Licensure & Certification (Required)

  • LICSW

  • Active DC Social Worker License

Job Responsibilities

  • Provide comprehensive Social Work services in accordance with licensure, certification, training, and regulatory requirements

  • Coordinate and facilitate services for patients during and after admission to the acute rehabilitation unit

  • Perform discharge planning, care coordination, counseling, and resource management to ensure safe and effective transitions across the continuum of care

  • Collaborate with interdisciplinary teams to deliver cost-effective and resource-efficient care

  • Respond effectively to new and changing work assignments while maintaining high standards of patient advocacy and compliance
