Title: Physical Therapist - Home Health
Location: San Jose, CA
Duration: 26 weeks
Shift: Day, 8:00 AM – 5:00 PM, 5x8, 40 hours, Monday–Friday
Start date: 01/12/2026

Requirements:

Experience: Minimum 1 year Home Health experience and 2 years overall as a clinician
Certification: Active State PT License, BLS, Current CPR Certification, Valid Driver's License
Charting System: Kinnser/WellSky
Patient Age Group: Geriatric

Experience: OASIS experience
