Title: Physical Therapist – Home Health
Location: Fredericksburg, VA
Duration:13-Week Contract
Shift: Day Shift | 5x8 hours | 8:00 AM – 5:00 PM

Job Responsibilities

• Provide one-on-one physical therapy services to adult and geriatric patients in a home health setting

• Evaluate patients, develop individualized treatment plans, and perform skilled therapy visits

• Complete a minimum of 24–30 patient visits per week to meet full-time expectations

• Manage an average caseload of 4–6 patients per day

• Document all patient care accurately in the electronic health record while in the clinic

• Communicate effectively with patients, caregivers, and interdisciplinary team members

• Maintain timely, complete, and compliant clinical documentation

• Travel within the assigned coverage area to perform home visits

Required Skills, Experience, and Qualifications

• Minimum of one (1) year of experience in home health or home care

• Valid state Physical Therapy license (must be in hand at submission)

• Current Basic Life Support (BLS) certification

• Experience providing therapy to adult and geriatric patient populations

• Strong time management, communication, and independent work skills

• Ability to meet productivity standards and complete documentation efficiently

Preferred Skills and Qualifications

• OASIS experience

• Experience with Kinnser/WellSky charting systems

• Acute care background

• First-time travelers accepted

• Self-starter with strong patient-centered care approach
