Title: Physical Therapist – Home Health
Location: Port Charlotte, FL
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 – 17:00)

Job responsibilities and all required and preferred skills and requirements:

• Provide in-home physical therapy services to geriatric patients across an assigned territory.

• Perform comprehensive evaluations, establish plans of care, and deliver skilled PT interventions.

• Complete accurate and timely documentation, including OASIS assessments.

• Utilize HomeCare HomeBase (HCHB) EMR for all clinical documentation.

• Manage an average caseload of 5–7 patients per day.

• Collaborate with interdisciplinary care teams to ensure quality patient outcomes.

• Travel within a defined territory (up to a 60-mile radius).

• Maintain professional standards, licensure, and compliance requirements.

Required Qualifications:

• Active Physical Therapist license in hand.

• Minimum 1 year of recent home health experience.

• Proven experience with OASIS documentation.

• HomeCare HomeBase (HCHB) EMR experience required.

• BLS certification.

• Previous charge experience required.

• Ability to work independently in a home health setting.

• Reliable transportation with proof of vehicle insurance.

Preferred/Additional Requirements:

• Experience treating geriatric patient populations.

• First-time travelers accepted.

• Comfort using provided iPad for documentation.

• Willingness to adhere to dress code (scrubs, no jeans).
