Title: Physical Therapist – Home Health
Location: Sturtevant, WI
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)

Job Responsibilities and Required/Preferred Skills:

• Provide home health physical therapy services, primarily for geriatric patients.

• Complete patient assessments, evaluations, plans of care, OASIS documentation, and timely follow-ups.

• Maintain all documentation by 9 AM the following day and communicate effectively with the team.

• Manage a patient load averaging 5–7 visits per day, meeting minimum productivity requirements (30 points weekly).

• Travel within a 60-mile radius of the hub/office as needed.

• Participate in on-call rotation every 3rd to 4th weekend.

• Use HomeCare Homebase (HCHB) EMR for charting and documentation.

• Follow facility dress code (scrubs, no jeans) and utilize iPad for patient care documentation.

• Mileage reimbursed at IRS standard rate.

Required Skills and Qualifications:

• Minimum 1 year of recent physical therapy experience.

• Proficiency in OASIS and HCHB EMR.

• Active PT license in-hand.

• BLS certification.

• Personal transportation and valid driver's license.

• Ability to work independently in home health settings.

Preferred:

• Previous home health experience.

• Flexibility with territory and patient scheduling.

• Experience managing timely communication and documentation in home health care.
