Description
Title: Occupational Therapist – Home Health
Location: Medford, OR (home health service area and surrounding regions)
Duration: 13 weeks
Shift: Day shift, 5x8 hours (08:00 – 16:30)
Job Responsibilities and Requirements:
Occupational Therapist will provide home health services to patients in their homes, delivering 1:1 care with up to approximately 6 visits per day while meeting productivity expectations. Responsibilities include evaluation, treatment planning, and implementation of occupational therapy interventions focused on improving independence, safety, and functional performance. Therapist will complete required documentation accurately and timely using the assigned charting system and participate in interdisciplinary care as appropriate. Reliable personal transportation is required due to travel between patient homes.
Required Experience & Qualifications:

Minimum of 1 year of skilled home health occupational therapy experience

OASIS experience required (must be clearly stated if not available)

BLS certification required
-
State OT license accepted pending at submission

Experience with charting systems (any system required)

Ability to work independently in a home health setting

Reliable transportation required

First-time travelers not accepted

Above- and below-knee prosthetics

Upper extremity prosthetics

Dynamic, functional, static, serial/inhibitory splints
