Title: Physical Therapist Assistant (PTA) – Skilled Nursing Facility (SNF)
Location: Los Angeles, CA
Duration: 13 weeks
Shift: Day Shift – 5x8-Hour (08:00 – 16:30)

Job Responsibilities

• Provide skilled physical therapy services to adult and geriatric patients in a skilled nursing facility (SNF) setting

• Assist with patient evaluations, develop individualized treatment plans, and set functional goals under the supervision of a licensed physical therapist

• Deliver therapeutic exercises, gait training, balance training, and mobility interventions

• Assist patients with transfers, use of adaptive equipment, and safe mobility

• Monitor patient progress and report changes in status to the supervising physical therapist

• Complete documentation accurately and timely using electronic charting systems

• Educate patients and caregivers on home exercise programs, safety, and mobility techniques

• Collaborate with interdisciplinary care teams to coordinate patient care

• Maintain compliance with infection control, safety, and professional standards

Required Skills & Experience

• Minimum of 1 year of experience as a Physical Therapist Assistant in a SNF, rehabilitation, or outpatient setting

• Experience with adult and geriatric populations

• Strong clinical assessment and care-planning skills

• Ability to work independently and manage patient caseloads

• Comfortable using adaptive and therapeutic equipment including walkers, crutches, slings, and prosthetics

Required Certifications & Licensure

• Physical Therapist Assistant license – State of California (required)

• BLS – Basic Life Support (required, active)
