Title: Physical Therapist – Home Health
Location: Saint Joseph, MO
Duration: 13 weeks
Shift: Day shift, 5x8 hours (08:00–16:30); 4x10 schedule may be available

Job responsibilities and all required and preferred skills and requirements

Home Health Physical Therapist responsible for providing comprehensive in-home physical therapy services to adult and geriatric patients across a multi-county service area. Duties include completing start-of-care admissions, evaluations, reassessments, and discharges; developing and updating individualized plans of care; and collaborating with PTAs and the interdisciplinary team. Therapist manages their own SOCs and provides patient and family education focused on safety, mobility, and injury prevention.

Position involves independent fieldwork with daily travel, flexible scheduling based on mileage and patient load, and participation in a rotating call and weekend coverage schedule.

Required Qualifications:

• Experience: Minimum 1 year of home health physical therapy experience

• Licensure: Active state physical therapy license

• Education: Doctor of Physical Therapy (DPT)

• Certifications: BLS required

• Charting System: Epic experience required

• Travel Experience: Required (home health preferred)

Clinical Skills & Competencies:

• Comprehensive assessments, evaluations, reassessments, and discharge planning

• OASIS documentation, plan of care development, goal setting, and time management

• Orthopedic, neurologic, cardiopulmonary, and geriatric rehabilitation

• Gait training, strength and mobility training, pain assessment, and vital sign monitoring

• Use of adaptive and assistive devices, transfers, and safety techniques

• Development and instruction of home exercise programs

Patient Populations & Conditions:

• Adults and geriatrics with general weakness, back pain, and orthopedic conditions

• Experience with post-operative orthopedic care, neurologic conditions, and chronic cardiopulmonary
