Title: Physical Therapist – Home Health
Location: Portland, OR (West Portland and surrounding areas)
Duration: 13 weeks
Shift: Day shift, 5x8-hour shifts (8:00 AM – 4:30 PM)

Job Responsibilities

• Provide skilled home health physical therapy services to adult and geriatric patients in their homes

• Perform comprehensive initial evaluations, reassessments, and discharge planning

• Develop, implement, and update individualized plans of care with short- and long-term functional goals

• Deliver therapeutic interventions including gait training, strengthening, mobility training, balance training, prosthetic training, and functional activities

• Complete OASIS documentation, visit notes, reassessments, and care coordination accurately and within required timeframes

• Educate patients and families on home exercise programs, medication awareness, safety, fall prevention, and injury prevention

• Assess home environments, identify environmental barriers, and recommend adaptive or assistive devices

• Perform safe lifting, transfers, and patient handling techniques

• Order necessary supplies and independently schedule patient appointments

• Manage time effectively while maintaining productivity expectations (1:1 visits; up to 6 visits per day; ~30 productivity points)

• Travel between patient homes using personal transportation; mileage is reimbursed

• Collaborate with interdisciplinary care team members to ensure continuity of care

Required Skills & Qualifications

• Minimum 1 year of skilled Home Health Physical Therapy experience

• OASIS documentation experience (required)

• BLS certification

• Ability to work independently in a home health environment

• Proficiency in:

  • Patient assessment, evaluation, reassessment, discharge, and documentation

  • Lifting and transfer techniques

  • Ordering supplies and scheduling appointments

• Experience treating patient populations including:

  • Adults and geriatrics

  • Dementia, Parkinson's disease, ALS, MS

  • Post-stroke rehabilitation

  • Orthopedic post-operative conditions

• Must have reliable personal transportation

• Willingness to float between facilities within the state if required

Preferred Skills & Experience

• Epic Home Health charting experience (Epic Remote Client and Rover/OASIS)

• Experience across multiple care settings: acute care, LTAC, outpatient clinics, rehabilitation hospitals, and skilled nursing facilities

• Manual therapy experience

• Hand therapy experience

• Wound care experience

• Billing experience
