Title: Physical Therapist Assistant – Home Health
Location: Portland, OR (West Portland and surrounding areas)
Duration: 13 weeks
Shift: Day shift, 5x8-hour shifts (8:00 AM – 4:30 PM)

Job Responsibilities

• Provide skilled home health physical therapy services under the supervision of a Physical Therapist

• Deliver individualized treatment interventions per established plans of care

• Treat patients in their homes on a 1:1 basis, with up to 6 visits per day and standard productivity expectations (~30 points)

• Perform therapeutic exercises, mobility training, gait training, balance activities, and functional task training

• Assist patients with transfers, ambulation, and use of assistive devices

• Monitor patient response to treatment and report progress or concerns to supervising PT

• Complete accurate and timely clinical documentation, including OASIS-related components as applicable

• Educate patients and caregivers on safety, mobility strategies, and home exercise programs

• Travel between patient homes using personal transportation; routes are generally grouped to minimize drive time

• Collaborate with interdisciplinary team members to support continuity of care

Required Skills & Qualifications

• Minimum 1 year of skilled Home Health PTA experience

• State Physical Therapist Assistant license (required)

• BLS certification

• OASIS experience (required)

• Experience providing care in a home health setting

• Ability to work independently in the field

• Reliable personal transportation

• Willingness to float between facilities within the state if required

Preferred Skills & Experience

• Experience with Epic Home Health charting (Epic Remote Client and Rover mobile app)

• Strong familiarity with home health workflows and productivity expectations
