Title: Physical Therapist – Home Health
Location: Saint Joseph, MO 64506
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 16:30), call required 1 Sunday every 5 weeks, weekends every 5th week (2 days off during the week and work Sat & Sun to complete admits)

Job Responsibilities and Required/Preferred Skills:

• Perform initial evaluations (SOC), reassessments, and discharges for home health patients; PTAs handle routine visits.

• Provide patient-centered care for adults and geriatrics with diagnoses including general weakness, orthopedic conditions (knee, hip, spine), post-surgical care, neurological conditions (stroke, TBI, Parkinson's), cardiopulmonary issues (CHF, COPD, CABG), oncology, and wound care.

• Administer therapeutic interventions including gait training, extremity mobilization, pelvic floor training, prosthetic training, spinal mobilization, modalities (E-stim, TENS, ultrasound, moist heat), and home exercise programs.

• Use adaptive and assistive devices, including Hoyer lifts, sit-to-stand lifts, wheelchairs, crutches, rolling walkers, shoulder slings, and specialty beds.

• Maintain documentation in EPIC EMR system (required).

• Provide patient and family education, safety/injury prevention, and monitor for signs of physical, sexual, or psychological abuse.

• Manage travel efficiently across 13 counties; adjust patient load based on mileage and travel time.

• Collaborate with interdisciplinary support teams including social services and unit secretaries.

Required Qualifications:

• DPT or PT license (pending license okay at submission; must be in-hand prior to start)

• BLS certification

• Minimum 1 year of home health experience

• Ability to travel and manage patients across multiple counties

Preferred Skills:

• Experience with EPIC charting system

• Familiarity with multiple payor requirements

• Previous travel experience
