Title: Physical Therapist – Home Health
Location: Chambersburg/Lebanon area, PA
Duration: 13 weeks
Shift: Day shift; typically 4x9.5-hour shifts (8:00 AM – 5:30 PM). Alternate schedules may include 9-hour days Sunday–Wednesday or 8-hour days Monday–Friday (8:00 AM – 4:30 PM).

Job Responsibilities

• Provide home health physical therapy services to patients in their residences

• Perform evaluations, develop and implement individualized treatment plans, and complete reassessments

• Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence

• Complete required documentation accurately and timely, including visit notes and care plans

• Coordinate care with interdisciplinary home health team members

• Educate patients and caregivers on safety, mobility strategies, and home exercise programs

• Travel between patient homes within the assigned service area and submit mileage documentation as required

• Provide care to patients with varying medical conditions, including possible COVID-positive patients

Required Skills & Qualifications

• Minimum 1 year of physical therapy experience

• Ability to work independently in a home health setting

• Willingness to work weekends as needed (varies by unit)

• Willingness to work at least one major holiday during the assignment

• Ability to meet documentation and productivity expectations

• Compliance with tobacco-free and fragrance-free workplace policies

• Ability to meet travel and mileage documentation requirements

Preferred Skills & Experience

• OASIS documentation experience (preferred, not required)

• Epic charting experience (heavily preferred)

• Prior home health experience
