Description
Title: Physical Therapist – Home Health
Location: Seaside, OR
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 16:30)
Job Responsibilities and Required/Preferred Skills:

Provide skilled home health physical therapy for adult patients in Seaside and surrounding areas.
-
Conduct patient evaluations, develop plans of care, perform home exercise program instruction, and document care in EPIC and OASIS.

Treat patients with orthopedic, neurologic, and general conditions, including post-op surgeries, spinal conditions, cognitive disorders, and post-stroke rehabilitation.

Apply body mechanics, assistive/adaptive devices, and educate patients and families on safety, injury prevention, and home exercise programs.

Travel to patient homes, with occasional drives up to 60 miles (60–90 minutes) per day; transportation required.

Manage patient load up to 6 patients per day (1:1 ratio) with a standard productivity expectation of 30 points.

Work independently in a home health setting without on-call or weekend requirements.
Required Certifications & Experience:

BLS
-
State Physical Therapy license
-
Minimum 1 year of skilled home health experience
Preferred Skills/Experience:

EPIC charting experience

Experience with OASIS documentation

Ability to work with diverse patient populations and provide comprehensive home health care
Work Setting: Home health environment with visits in patient residences; the system minimizes drive time by grouping clients efficiently.
