Description
Title: Physical Therapist – Home Health
Location: Ketchikan, AK
Duration: 13 weeks
Shift: Day shift, 4x10-hour shifts (8:00 AM – 6:00 PM); self-scheduled
Job Responsibilities
-
Evaluate home health patients and develop individualized treatment plans and goals.

Provide skilled physical therapy services in patients' homes, adjusting care plans based on patient progress and needs.

Complete accurate, timely documentation, including evaluations, reassessments, discharges, plans of care, and goal setting.

Perform and document OASIS assessments as required.

Educate patients and families on home exercise programs, safety, injury prevention, and disease management.

Monitor and assess vital signs, pain, strength, sensory response, and functional mobility.

Identify environmental barriers and recommend appropriate adaptive or assistive devices.

Collaborate with interdisciplinary teams, including social services and interpretation services, to ensure coordinated care.

May provide oversight or mentoring to therapy assistants, aides, or students.
-
Recognize and report signs of physical, sexual, or psychological abuse per policy.
-
May float to hospice within scope if needed.
-
Perform additional duties as assigned to support quality patient outcomes.
Required & Preferred Skills and Requirements
Licensure & Certifications (Required):

State Physical Therapist License (compact accepted; pending license not accepted)

BLS (Basic Life Support)
Experience:

Minimum 1 year Home Health / OASIS experience required

First-time travelers accepted if OASIS experience is present
-
Travel experience not required
Charting System:
-
Epic preferred
Work Setting:
-
Home Health (community-based care in patient homes; office on campus)

Reliable vehicle required
